Who Should Complete This Form?

Undergraduate Students
Must have completed at least 80 credits towards their program.
Note: Undergraduate students pursuing BS Elementary Education and BEd Primary Education must have completed a minimum of 115 credits hours towards their program.

Graduate Students
Must have successfully completed the first year of their program.
If enrolled in a one-year program, must be in the second semester.  

Previously Submitted
If you submitted this form last year but were not eligible to graduate, you must submit it again to be considered for 2026 graduation.

Important Notes:
You only need to submit this form once for the 2026 graduation cycle. After submission, the Academic Auditor will contact you in August/September with your academic audit.
